Not enough "realistic" veins, eye crust and possible symptoms of pink-eye in his eye sockets, plus he could use a LOT more "realistic" canker sores, pus-filled acne and ingrown moles due to debilitating skin cancer, but otherwise, yeah, this is likely what Hollywood would turn him into, just to flex that their computers can handle excessively-detailed, unoptimized 32K textures while yours can't.
